The Hamilton Wharton Group Inc. is the designated agent for an association-sponsored medical insurance program called NY Healthcheck. NY HealthCheck serves the health insurance needs of New York businesses with 51-250 employees. By joining NY HealthCheck, these businesses enjoy lower health insurance premiums.

Working with The Hamilton Wharton Group Inc., the Association of New York Health Care Facilities Inc. teamed up with a major health insurance carrier to bring NY HealthCheck to the marketplace. The Association of New York Health Care Facilities is a not-for-profit, non-partisan organization whose mission includes lowering costs for its members. Our insurance carrier is one of the largest health care providers in New York with more than 20,000 physicians and other providers in its network.

NY HealthCheck offers a full menu of POS, EPO, and PPO benefits plan selections. Eligible employees may choose a healthcare plan that fits their medical needs and budgets from an employer-driven range of choices.

To join NY HealthCheck, a company must be located in New York State and be a member of the Association of New York Health Care Facilities.

The following information is required to obtain a quote.

1.	Completed Employee Census Form
2.	Loss history with detailed claimant data
3.	Copy of the most recent bill with detailed premium information
4.	Plan design summary from current carrier
5.	Drug plan summary from current carrier
6.	Copy of the renewal letter with current and renewal rates
7.	Employer contribution (i.e. percentage or dollar amount)
8.	Eligibility requirement (waiting period before a new employee is eligible for coverage)
9.	Tier rating (i.e. 2, 3, or 4 tier)
10.	Copy of the most recent NYS-45 Form
